In the spring of 1976, two years into my psychiatric practice, I began having pain in both knees, which soon severely limited my running. I was advised by an orthopedist to stop trying to run through the pain. After many failed attempts to treat the condition with orthotic surgery and physical therapy, I resigned myself to giving up running. As soon as I made that decision, the fear of gaining weight and getting fat consumed me. I started weighing myself every day, and even though I was not gaining weight, I started feeling fatter. I became increasingly obsessed about my energy balance and whether I was burning off the calories I consumed.
